    MC-FP Alternative Activities—Part IV

    ABFM Diplomates who complete Part IV alternative activities must submit the required MC-FP fees in order to receive MC-FP credit for the activity. In addition, for the alternative activity to apply toward the Diplomate's MC-FP Part IV requirement, it must be completed during the stage in which the Diplomate wishes to receive the credit.

    Notification of Participation in Approved Part IV Alternative Activities  

    The method for notifying the ABFM of Diplomates’ successful completion of the requirements of the approved activity is determined by the external provider and is typically achieved in one of three ways: the external provider can submit completion data to the ABFM on behalf of Diplomates on a monthly basis; the external provider can issue a certificate of successful completion (the Diplomate must mail or fax a copy of the certificate to the ABFM in order to receive credit); or the external provider can submit a Physician Attestation form for each participant (the attestation must be completed and signed by the Diplomate and must be cosigned by the external provider administrator). Within 2 weeks of receipt of notice of successful participation in an approved alternative activity, the ABFM will update the Diplomate’s Physician Portfolio.

    • American Academy of Family Physicians (AAFP) activity Improving Evidence-based Primary Care for Chronic Kidney Disease [Link]. Participation is limited to Diplomates who have an agreement with the AAFP to participate in this project at the practice level.     • Aurora Health Care activities Asthma Control Test in Treating Asthma and Nutrition Approaches for the Management of Chronic Disease and Diabesity [link].
