    Alternative Activities—Performance Improvement

    The ABFM has approved many alternative activities in which Diplomates can participate to earn Performance Improvement credit. In order for an alternative activity to apply toward a Diplomate's Performance Improvement requirement, it must be completed during the stage in which the Diplomate wishes to receive the credit. Diplomates who complete approved alternative Performance Improvement Activities still must submit any certification fees that may be due in order to receive credit for the activity toward Family Medicine Certification Requirements.

    Notifying the ABFM of Participation in Approved Alternative Activities
    The ABFM is notified of a Diplomate's successful completion of the requirements of an approved activity in one of three ways (to be determined by the activity sponsor):

    • the activity sponsor submits completion data to the ABFM on behalf of Diplomates on a periodic basis;
    • the activity sponsor issues a certificate of successful completion that the Diplomate must mail or fax a copy to the ABFM; or
    • the activity sponsor submits a Physician Attestation form for each participant (the attestation must be completed and signed by the Diplomate and must be cosigned by the activity sponsor administrator)

    Usually, the ABFM will update the Diplomate's Physician Portfolio within two weeks of receipt of notice of successful participation in an approved alternative activity.

    Submitting Participation Information to the ABFM
    Participation information (i.e., completion data reports, completion certificates, physician attestations) should be mailed, faxed, or emailed to:
